LIVEWIRE_GPIO_SLOTS Table Layout for Rivendell

This table holds the 'mapping' between LiveWire source numbers and
the Rivendell GPIO 'slots' used by the LiveWire Virtual GPIO driver.
Each slot consist of five Rivendell GPIO lines.
